First, they should ask about their chronic medical conditions, medications, and medical history/surgeries, including previous episodes of electrolyte imbalances or acid-base disturbances. Second, they should conduct a physical examination, including edema, dry mucous membranes, skin turgor, neurological changes, and vital signs, and assess for signs of dehydration or overhydration and symptoms like breathing difficulties, muscle cramps, weakness, palpitations, nausea, vomiting, and changes in urinary patterns (UNC School of Medicine, n.d.). Next, they should order laboratory tests for serum electrolytes, BUN, ABGs, ECG, and urine electrolytes and note daily fluid intake and output (UNC School of Medicine, n.d.). Also, review the medications to result in electrolyte imbalances and assess dietary habits and nutritional status.

Moreover, an NP should educate patients and families to maintain fluid and electrolyte balance by modifying dietary habits and fluid intake.

  • Assess for signs of dehydration, such as dry mucous membranes, sunken eyes, and poor skin turgor.
  • Look for signs of overhydration, including peripheral edema and pulmonary congestion.
• Laboratory Tests:
  • Order appropriate laboratory tests, such as serum electrolyte levels (sodium, potassium, calcium, magnesium), blood urea nitrogen (BUN), creatinine, arterial blood gases (ABGs), and complete blood count (CBC).
  • Assess urinary electrolyte concentrations and osmolality to evaluate the renal response to fluid and electrolyte imbalances.
